Third Times a Charm – Mentor Ridge Earns Third Consecutive Deficiency Free Survey from Ohio Department of Health

Mentor Ridge Health and Rehabilitation is proud to announce it received its third consecutive Deficiency Free survey from the Ohio Department of Health (ODH). Since the facility opened in 2017, it has not had a citation issued during the state's annual survey.
Earning a Deficiency Free survey is hard work, as the average number of citations issued during an annual survey in Ohio is around 7. To earn three Deficiency Free Surveys in a row is remarkable.
This accomplishment is one of many notable achievements that Mentor Ridge has earned within the last year. They were also voted as being in the top 2 percent of nursing homes for family satisfaction, according to the Ohio Department of Aging. And, the Centers for Medicare and Medicaid (CMS) rated Mentor Ridge 5 stars—the highest rating— in quality measures, health inspection, and overall rating.
The staff at Mentor Ridge are excited to celebrate their accomplishment with their residents, who are the reason they work so hard to achieve great things. When administrator Chuck Hillier was asked how he lead the team to its third straight deficiency-free survey, he said it was as simple as following Mentor Ridge's mission of having "The Right People doing the Right Thing for the Right Reasons."
"This was business as usual for us," Chuck said. "We give 100 percent effort every day, so this week was no different. It is nice that our team was recognized for their hard work with this outcome, but we would have done it regardless because it's the right thing to do."
